Acquire Insurance for Equipment on loan to GT for Official Business

• Complete the Non-GT Owned Epuipment Form.
• Fax or mail completed form and loan agreement to Insurance & Claims Management.
• Insurance is initiated when form and loan agreement is received by Insurance & Claims Management.

Acquire Insurance for GT Owned Equipment and Non-GT Equipment when shipped via Commercial Carrier by Georgia Tech

• Complete the Trip Transit Insurance Form.
• Email, fax, or mail completed form to Insurance & Claims Management.
• Insurance is initiated when form is received by Insurance & Claims Management.
