File a Fidelity Bond Claim

  • Report employee dishonesty that results in loss of Institute property to GT Police.
  • Report incident to Insurance & Claims Management by email or fax within 24 hours.
  • The State Self-Insurance Program determines whether a claim is covered.

File a General (non-Auto Related) Liability Claim

  • Report injury/property damage that occurs on campus to GT Police immediately.
  • Report incident to Insurance & Claims Management by email or fax within 24 hours.
  • Email or Fax estimates of damage and/or medical bills to Insurance & Claims Management.
  • The State Self-Insurance Program determines whether a claim is covered.

File a Property Insurance Claim

  • Report property damage or loss to GT Police immediately.
  • Report damage/loss to Insurance & Claims Management by email or fax within 24 hours.
  • Obtain estimates of damaged property and email or fax to Insurance & Claims Management.
  • The State Self-Insurance Program determines whether a claim is covered.
  • Any Insurance recovery received for a claim will be credited to your account via journal entry.
  • Replace or repair damaged or loss property.

File a Workers' Compensation Claim

  • Report injury to Supervisor.
  • Supervisor report injury to workers' compensation toll free number: 1-877-656-7475

  • Claims should only be reported by the Supervisor.
  • Injuries should be reported within 24 hours.
  • Only injuries requiring medical care or lost time should be reported.
  • Injuries not requiring medical care (e.g. minor cut, scrape, burn, etc.) or involving lost time should be recorded on an “Incident Notice Only” form and kept with the employee’s personnel records
  • The State Self-Insurance Program determines whether a claim is covered.
